Referrals From Physicians Outside the Sharp System or Not Using Epic
If you are not part of the Sharp network or do not use Epic:
Fax the completed referral form to:

Once received, our team will:
- Review the referral for urgency and clinical details
- Contact the patient promptly to schedule the appropriate appointment
- Communicate recommendations back to the referring provider
Referrals From Sharp Providers Using Epic
If you are a Sharp-affiliated provider using Epic, please place your referral directly through the EMR.
This allows:
- Direct routing to our scheduling team
- Immediate access to clinical documentation
- Faster coordination and fewer administrative delays
Our team will follow up with your patient and ensure all results and recommendations are returned through the EMR.
Information to Include With Every Referral
Including the following information helps us schedule the correct appointment type and ensures timely care:
- Reason for referral (e.g., abnormal ultrasound, medical complication, screening result, multiple gestation)
- Relevant clinical notes
- Recent ultrasounds and imaging
- Laboratory or screening results (including prenatal screening/NIPT if applicable)
- Insurance information
- Patient contact information
Providing these details upfront helps reduce delays and improves the patient experience.
